New Patient Forms
Again, welcome to the Sleep Health Center,
("SHC")!
Our New Patient Forms that will download
after you click on each of the Forms buttons below are in
a "fillable" .PDF file that cannot be submitted electronically
from this Web site.
Instead, you must
print out the appropriate forms and bring them with you on
your next visit to either the doctor's office
or the Sleep Health Center, but please
continue reading this page.
●NOTE: After
you click on and try to type into the first
fillable data entry field, there will be a
delay of 30 seconds until Adobe Reader
displays a message box - close the message
box and you
can proceed to enter data.
* Note that below there are two
different sets of forms you can
download:

Doctor
Visit New Patient Forms (19 pages) - for your first visit to
the doctor's office.

SHC Sleep
Session Forms (5 pages) - for if your doctor prescribes a
sleep session at the Sleep Health Center. Until a sleep session
is prescribed for you, you don't need to print these forms
unless you want to learn about the in-depth questions that we
use to help diagnose and treat sleeping disorders.
Our new "fillable" forms provide you the
convenience of using your computer to type necessary information
directly into these forms before
you print them out to bring them to our
office for your first visit. This saves time
for both you and our office staff, and
offers far higher accuracy in capturing your
information correctly.
After you have downloaded the forms and
are viewing the forms on your computer
monitor,
if data entry fields where you can type
necessary information are not
highlighted, there may be
a small check box in the upper right hand
corner of the form's window with the words
Highlight fields that you can
click to cause the data entry fields to
become highlighted.
